Documents and Forms for Accepted Students

Acceptance to the Bachelor of Science in Nursing program is not deemed final until the required documents below have been submitted prior to the commencement of the program.

Basic Life Support certification

All students in the nursing program are required to have current certification through a BLS (Basic Life Support) provider. Certification must take place in an instructor-led, in-person course, not online. You must upload a copy of your certificate showing the effective date to the required documents Moodle page for your class.

WHMIS

All students in the BScN program are required to have a Workplace Hazardous Materials Information System (WHMIS) Certificate. This course is available through TRU (via Moodle), in your community or online. Once successfully completed, upload a copy of the WHMIS Certificate to the required documents Moodle page for your class.

Student immunization record

All TRU nursing students visiting health care service delivery sites for clinical placements are considered health care providers and should be protected against vaccine preventable diseases. You must follow provincial and practice agency immunization guidelines and policies. Such policies are based on the Communicable Diseases and Immunization Guidelines from the BC Centre for Disease Control and the Canadian Public Health Agency. You must upload a copy of your completed form to the required documents Moodle page for your class.

HSPnet form

Disclosure of student information. You must upload a copy of your completed form to the required documents Moodle page for your class.

BCCNM requisite skills and abilities

  1. Review the BCCNM requisite skills and abilities.
  2. Sign the TRU BScN Requisite Skills and Abilities form indicating that you have reviewed this document.
  3. Upload a signed copy of this form to the required documents Moodle page for your class.

Consent to a criminal record check

All students in the BScN program are required to completed a criminal record check. Complete the consent form and upload the form, along with two pieces of photo ID to the required documents Moodle page for your class. One of the pieces of identification must be government issued and include the applicant's name, date of birth, signature and photo. If you do not have two pieces of photo ID you can have the consent form notarized. Payment is not required, the charge will be included in your first practice course.
