New Blackberries and Cell Phones

1. A Service Agreement form needs to be filled out (make, model & cost of cell phone) and signed by Department head in order to authorize the purchase of the blackberry/cell phone.  Service Agreement forms are available from the dealer.  Vendor will activate the blackberry/cell phone once the form is completed, signed and a PO number is issued.

2. Send a Web Requisition  to Purchasing requesting the purchase of a new blackberry/cell phone service from either Telus Mobility or Rogers.  Include the make, model and cost of the phone on the PR.  (send a copy of the service agreement if possible)

3. Vendor will need to contact David Burkholder at (250) 828-5324 (TRU's IT Services)  at the time of activation, to get the account password.

4. Vendor is to send an invoice copy to Purchasing.  Fax (250) 371-5512.
