Applications and Systems Access Request (ASAR) form
Purpose:
The ASAR form must be completed for all TRU faculty, staff, and approved contract staff, to be granted TRU network access, as well as for access to specific applications required to carry out their work. This form is not intended to be used for student accounts.
Please submit this form as soon as you know you have a new employee starting. IT Services should be notified as soon as possible if a new computer and /or telephone is also required for the position.

and mail, fax, or deliver it to IT Service desk in BCCOL. Note we now have our own fax number (250-852-7292) The form can also be scanned and emailed to the ITServicedesk@tru.ca

Banner accounts Information
Complete the Applicant information:
Include Name, Employee number, Job Title, Division, Department, Location and Building/Room number, and Phone Number, The Access start date which should be the employee’s first day of work or the date of the additional Application/System request. If the staff member is only hired for a specified period, the End date must be noted. A new form to extend all access should be submitted if the applicant is extended. Check a box for the type of employee (i.e. CUPE, TRUFA, etc.). Place a checkmark next to the applications that you wish to have access to. Include any additional information in the Access Details section to help us provide you with the correct access, first try. It is important that the form is completed in full otherwise there may be a delay in account creation.
Managers/Supervisor’s signature:
Once the form is completed, the Supervisor will sign that they have reviewed all of the requested Application/System accounts to acknowledge that these are part of the job requirements and to ensure that the policy has been/will be read and understood by the applicant. The applicant signature can be completed when the employee starts. Do not hold the form until the employee actually starts. Account information will be provided to the Manager/Supervisor listed on the form.
Data Owner’s signatures (required)
The Data Owner signatures (as noted) must be gathered before those accounts can be created. Please do not submit the form without those signatures,
Processing Turnaround:
Accounts may take up to two weeks to fully process, provided all signatures and access details are included.
